Ventricular tachycardia and fibrillation with implanted electrical pacemakers.

چکیده
The treatment for patients with Stokes-Adams attacks complicating complete heart block now includes implantation of electrical pacemakers (Elmqvist and Senning, 1960; Glenn et al., 1959; Simpson et al., 1962). With increasing clinical experience, complications of this form of treatment are becoming more apparent. These include component failure with resultant inadequate stimulation (Chardack, Gage, and Greatbatch, 1961; Zoll et al., 1961; Elmqvist et al., 1963), or ventricular tachycardia and fibrillation with normal pacemaker function (Elmqvist et al., 1963; Portal et al., 1962; Dressler, 1964; A. Leatham and J. G. Davies, 1964, personal communication). Dressler (1964) reviewed ventricular arrhythmias in patients with normally functioning pacemakers and considered that a slow rate of stimulation was the underlying cause of these arrhythmias. We report 3 patients with recurrent ventricular tachycardia and fibrillation in the presence of normal pacemaker function.

The recently published 2008 Guidelines for Device-Based Therapy of Cardiac Rhythm Abnormalities1 include a discussion of ethical issues surrounding the deactivation of pacemakers, implanted cardioverter-defibrillator (ICDs), and CRT devices in patients who are terminally ill. The authors rightly note that most clinicians make an ethical distinction between deactivating an ICD and deactivating a...

PURPOSE Recent studies have demonstrated that ventricular rate control is a viable treatment strategy for patients in atrial fibrillation (AF). The purpose of this study was to determine whether or not the benefits of coupled pacing (ie, a proposed rate control therapy) could be used during persistent AF. DESCRIPTION Six mongrel dogs were chronically implanted with endocardial atrial and vent...

متن کاملProarrhythmic effect of pacemaker stimulation in patients with implanted cardioverter-defibrillators.
BACKGROUND We sought to determine the potential of right ventricular VVI backup pacing to induce ventricular tachyarrhythmias in patients with implanted cardioverter-defibrillators. METHODS AND RESULTS All consecutive patients presenting exclusively with pacemaker-induced tachycardias (PITs) were included in a prospective study using a crossover protocol. Patients were randomized to either gr...
متن کاملReuse Potential of Pacemakers and Implantable Cardioverter-defibrillators - Some Real World Data
Patients who survive either ventricular fibrillation or sustained ventricular tachycardia carry a high risk of further episodes, which could be fatal. Anti-arrhythmic drugs were the standard treatment for patients with malignant ventricular arrhythmias, but despite using the best appropriate medical treatment, arrhythmia recurrence rates remain high at 40-50% at five years. Implantable cardiove...
